LangGraph Postgres setup(): CREATE INDEX CONCURRENTLY cannot run inside a transaction block
Calling `.setup()` on LangGraph's `PostgresStore` (or `PostgresSaver`) with a plain `Connection.connect(conn_str)` fails, because the setup migrations use `CREATE INDEX CONCURRENTLY`, which PostgreSQL does not allow inside a transaction, and psycopg opens transactions by default.

Open the connection with `autocommit=True` (and `row_factory=dict_row`), or use `from_conn_string`, which already handles this:
```python
conn = Connection.connect(conn_str, autocommit=True, row_factory=dict_row)
store = PostgresStore(conn)
store.setup()
```
A LangGraph maintainer confirmed `from_conn_string` handles it and pointed to the README callout.
