MCP OAuth sign-in fails with issuer mismatch (RFC 9207)
After the browser sign-in for a remote MCP server, the client fails the login because the `iss` parameter on the redirect does not match the issuer advertised in the server's OAuth metadata. A wrong issuer at this step is what an authorization-server mix-up attack looks like, so the client refuses to exchange the code.

1 solution
This is a server-side fix: the authorization server must return the same issuer in the `iss` parameter that it advertises in its metadata. If metadata sets `authorization_response_iss_parameter_supported`, the redirect must include `iss`. As a client, retry once, then report it to the server operator; `MCP_SDK_GENERATION=v1` bypasses the check but removes mix-up protection.
