MCP OAuth helpers: ReferenceError crypto is not defined
OAuth flows in the MCP TypeScript SDK crash with a ReferenceError. The OAuth client helpers use the Web Crypto API at `globalThis.crypto`, which is always defined on Node.js 20+ (required by every `@modelcontextprotocol/*` package) but not on Node.js 18 and earlier.

Upgrade to Node.js 20+. If you cannot, install the polyfill before anything touches the SDK:
```ts
import { webcrypto } from 'node:crypto';
if (typeof globalThis.crypto === 'undefined') {
globalThis.crypto = webcrypto;
}
```
