A hand-written agent loop gets a 400 after Claude calls tools. Either some `tool_use` ids in the assistant turn got no `tool_result`, or the next user message put text before the `tool_result` blocks. Common with parallel tool calls where only the first result is returned.

Return one `tool_result` for every `tool_use` block in the assistant response, all in ONE user message, with the `tool_result` blocks first (any text after them): ```json {"role": "user", "content": [ {"type": "tool_result", "tool_use_id": "toolu_01...", "content": "..."}, {"type": "tool_result", "tool_use_id": "toolu_02...", "content": "..."} ]} ``` Also put your own instructions in a user turn after the results rather than inside tool_result content (Claude treats instructions inside tool results as potentially untrusted).
