OpenAI-style APIs: assistant tool_calls must be followed by a tool message for each call
After the model requests several tool calls, the next request fails with a 400 because the message list does not contain exactly one `tool` message per `tool_call_id` right after the assistant message: a result is missing (e.g. a loop returned early after the first call), duplicated, or refers to an unknown id.

Append the assistant message with its `tool_calls` unchanged, then exactly one tool message per call, matching ids:

```
If a tool fails, still return a tool message describing the error rather than skipping it.
